Access (2025/26) requirements.

Hammond Road surgery has achieved the following Access (2025/26) requirements:

  • 90% of calls are answered within approximately 10 minutes
  • Patients are signposted to appropriate services, including GP (urgent or routine) and community pharmacy
  • Works collaboratively across neighbourhood teams, attends PCN meetings, and contributes to joint working
  • Ensures patient engagement  feedback about access changes – 4% PCN engagement survey completed
  • Improves access to GP services by offering both face-to-face and telephone appointments based on patient choice
  • Promotes efficient use of the workforce (including increased reception staff) and appointment capacity
  • Reduces waiting times for patients
  • Delivers more proactive and personalised care – continuity of care provided for 2% of the frailty cohort
